Arizona 66033 <br />f602~ 6773201 <br />RE: Annual Embankment Inspection - Seneca II Mine <br />Stipulation 5 to Permit No. C-80-005 <br />Dear Mr. Mount: <br />Stipulation 5 to Permit No. C-80-005 is as follows: <br />"The embankment must be inspected annually, or within 10 <br />days following any storm whose backwater elevation exceeds <br />the elevation of 6941.1 feet above sea level (10-year, <br />24-hour maximum storm event's projected backwater <br />elevation), by a qualified Geotechnical Engineer or <br />Engineering Geologist. A letter of certification containing <br />the results of the inspection must be submitted to the <br />Division within two weeks following the inspection." <br />In accordance with this stipulation, I, Ronald J. Gehrke, <br />Colorado P.E. 19959, have inspected the subject embankment <br />on May 23, 1990 and do herewith submit my report of <br />findings: <br />I. Chronology <br />A. July 14, 1981: F.M. Fox and Associates submitted <br />a report on their geotechnical slope stability <br />investigation of the embankment. The report <br />concluded that the embankment was stable with a <br />minimum static safety factor of 1.52. The report <br />also recommended revegetation and inspections. <br />B. September 23, 1981: Colorado Mining Permit No. <br />C-80-005 became effective. <br />C. Fall 1981: The embankment was hand seeded in <br />compliance with Stipulation 16 of Permit No. <br />C-80-005. <br />D. June 15, 1982: The annual embankment inspection <br />was conducted. Weather conditions were favorable, <br />skies partly cloudy, temperature about 65° F. A <br />light rain had fallen on the evening of June 14th. <br />
