Waiver for Mining Activities within alb' of Occupied Dwelling
Dear Mr. and Mts. Iuppa:
Under applicable regulatory provisions, mining and reclamation activities aze prohibited within 300' of
occupied dwellings without the written consent of the effected property owner. Based on our previous
discussions and ongoing cooperative relationship, it is our understanding that you fully suppoR and
encourage Energy Fuels Mining Company's (EFMC) mining and related activities at We Raton Creek Mine
(formerly Animas/Helen Mines). It is also our understanding that you would like to see mining activities
continue, to the extent that such activities are economically feasible, and that you desire that site reclamation
be completed consistent with the approved plans upon completion of mining.
In order to verify that you understand that you have a legal right to deny mining activities within 300' of
your dwelling, that you knowingly waive that right, and that you support end consent to EFMC's ongoing
mining and reclamation activities please sign this waiver statement below and return one signed copy in the
enclosed envelope.
