Re: Mid-Term Review of the Southfield Mine, Permit No. C-81-014
Dear Mr. Weaver:
Enclosed is the Division's mid-term review and decision to require revisions
to the Southfield Mine Permit No. C-81-045. The review is in fulfillment of
Rules 2.08.3, 2.06.2(9), 2.06.3(4), 2.06.5(3), 2.06.7(5), and 3.02.2(4). Also
• enclosed are enforcement actions that the Division is taking as a result of
the mid-term review.
The mid-term review consisted of reviewing the permit application, stipulation
responses, revisions and any supplemental information, as well as, information
from field inspections an past inspection reports. The main purpose of the
review was to identify any areas of the permit application that will need to
be revised or modified to ensure compliance and reflect the current status of
the mine.
The enclosed document contains out mid-term review, a rational for requiring
any revisions to the application, a summary of the status of stipulations, and
the results of our performance bond review.
Revisions were found under the categories of Legal, Financial and Compliance
information, Subsidence, Operation Plans and hydrology and hydrologic
monitoring.

The Division requests that the Southfield Mine submit appropriate responses to
these concerns by March 15, 1987. The responses should be submitted in a
package using a format that allows easy insertion of the responses directly in
to the original permit application.
