Dear Mr. Camilletti:
Listed below are adequacy review comments on the above-captioned Regular 112 Application for 45
permitted acres located within Routt County. The decision date for your application is scheduled for June
28, 2001. Therefore, I would appreciate a response from you on or before June 15, 2001. Please be
advised that your application may be deemed inadequate and may be denied on June 28, 2001 unless the
following adequacy items are addressed to the satisfaction of the Division. If additional time is needed to
prepare and submit an adequacy response, the applicant may waive [heir right to a decision within 90 days
and request an extension of time. However, such request must be made in writing and will need to be
received by the Division on or before June 15, 200].
6.4.3 EXHIIi[T C -Pre-Minine &Minine Plan Nlap(s) of Affected Lands
I. The location of the permit boundary as depicted on Map 3, Mine Plan Map is not clear. Please clarify
the 45-acre permit boundary on all maps provided, in accordance with Rule 6.4.3(d).
2. Although not explicitly required, any aerial photographs of the proposed project and surrounding area
including Trout Creek and the Yampa River will be helpful. Please provide if available.
3. On page 6, the applicant references Map #5, with the location of the existing Clark Ditch. Please notz
that Map #5 was not provided with the application. In conformance with Rule 6.4.3(f), the Division
requests that the applicant provide a water features map with the following items: Clark Ditch, any other
irrigation ditches or laterals, any springs, any stockwater ponds, any wells on or within 600 feet of the
proposed excavation, Trout Creek, Trout Creek 100-year floodplain. Trout Creek Floodway, Yampa River,
Yampa River 100-year floodplain, and Yampa River Floodway. Please provide.
6.4.4 EXHIBIT D -Minine Plan

4. The applicant specifies on page Z of the application form that an asphalt batch plant may be installed in
the future. Please note that a permit modification (technical revision or amendment) will need to be
accepted and approved by the Division before such use.
