History Well 21-0 <br />• The silica sand filter pack was placed from 1420 to 1452 ft. Bentonite hole <br />• plug was placed immediately above and below the sand filter pack at 1405 <br />to 1420 ft and 1452 to 1477 ft, respectively. All annular fill materials were <br />placed via the 2.19-inch-OD tremie line. <br />• The Dissolution Zone well casing was constructed with a string of Centron <br />2.375-inch 1.29 Ib/ft, upset end integral box fiberglass casing in nominal <br />29-ft lerigths from surface to 1432 ft. <br />B Groove <br />• The annular interval above the Dissolution Zone well and below the B <br />Groove installation, from 1109 to 1405 ft, was sealed with 177 sacks of <br />Class A Type II cement with -2% calcium chloride. The cement was <br />placed via tremie pipe in three separate vertical lifts. <br />• The B Groove well (21~B) was installed after the cement adequately <br />cured, and completed with a 10-ft fiberglass, #20 slot screen at 1076 to <br />1086 ft in ahigh-porosity zone near the center of the B Groove. <br />The silica sand filter pack was placed from 1065 to 1093 ft. <br />• Bentonite hole plug was placed immediately above and below the sand <br />filter pack at 1008 to 1032 ft and 1093 to 1109 ft, respectively. All annular <br />fill materials were placed via the 2.75-inch-OD tremie line <br />• The B Groove well casing was constructed with a string of Centron 2.375- <br />inch, 1.29 Ib/ft, upset end integral box fiberglass casing in nominal 29-ft <br />lengths from surface to 1086 ft. <br />The annular interval above the B Groove well and below the A Groove <br />• installation, from 886 to 1008 ft, was sealed with 62 sacks of Class A Type <br />II cement with -2% calcium chloride. The cement was placed via tremie <br />pipe in two vertical lifts. <br />A Groove <br />The A Groove well (21-4A) was installed after the cement adequately <br />cured, and completed with a 10-ft, Schedule 80 PVC, #20 slot screen at <br />862 to 872 ft in ahigh-porosity zone near the center of the A Groove. <br />The silica sand filter pack was placed from 855 to 879 ft. <br />• Bentonite hole plug was placed immediately above and below the sand <br />filter pack at 845 to 855 ft and 879 to 886 ft, respectively. All annular fill <br />materials were placed via the 2.75-inch-OD tremie line. <br />The A Groove well casing was constructed with a string of 2-inch-OD, <br />Schedule 80, flush joint PVC casing in nominal 20-ft lengths from surface <br />to 862 ft. <br />• The annular interval above the A Groove well and below the lower Uinta <br />Formation installation, from 675 to 845 ft, was sealed with 80 sacks of <br />Baroid Aquaguard grout. The grout was placed via tremie pipe in one <br />continuous vertical lift. <br />Well Development <br />• Developed well by air-lifting and bailer. Removed the following volumes <br />prior to sample collection: <br />. 21-4A 984 gallons <br />21-46 1,100 gallons <br />21-4D 985 gallons <br />
