Bentonite hole <br /> plug was placed immediately above and below the sand filter pack at 868 <br /> to 898 ft and 923 to 948 ft, respectively. The grout was placed via tremie pipe in one <br /> continuous vertical lift. <br />• The lower Uinta Formation well (21-3U) was installed after the grout <br /> placement, and completed with a 10-ft, Schedule 80 PVC, #20 slot screen <br /> at 480 to 490 ft in ahigh-porosity zone, <br />• The silica sand filter pack was placed from 472 to 495 ft. <br /> Bentonite hole plug was placed immediately above and below the sand <br /> filter pack at 462 to 472 ft and 495 to 509 ft, respectively. All annular fill <br /> materials were placed via the 2.19-inch-OD tremie line. <br /> <br />
