Discharges From the Valley Leach Facility Thar Qualh~ for Annual Precipitation or Storm Exemptions <br />The permittee shall provide verbal notification within 24 hrs of its discovery, and submit written <br />notification to the Division within 5 days of its discovery, a description of any discharge from the Valley <br />Leach Facility that may quay for the annual precipitation or storm exemptions described in sections <br />VI.A.2 and 3. The permittee shall submit in thrs notification a description of what measures will be taken <br />to control or eliminate the discharge. <br />Within 10 days of its discovery, the permittee shall submit to the Division the results of the first laboratory <br />analysis of any discharge subject to this not~catron requiremem. Unless directed differently by the <br />Division, all subsequent laboratory results shall be submitted as an attachment to the monthly Discharge <br />Monitoring Report for the month during which the discharge occurred. <br />After any discharge subject to these provisions has been eliminated, the permittee shall submit notification <br />to the Division that such discharge has ceased, within 5 days of the termination of the discharge. <br />c. The permittee is required to follow the notfcation requirements which apply to points 002A and 003A, <br />and which are explained in the e,[fluenr limit and monitoring condition sections of this rationale. The <br />permittee shall provide verbal notfcarion within 24 hrs, and written notification within S days. <br />d. Annual Report <br />The permittee shall submit, by February 2817t of each year, an annual report which will include the <br />following information: <br />(1 J For the potential drscharges associated with the numbered drscharge points specifically ideas fled in <br />the tables of effluent limitations and monitoring conditions li-rted in sections V1. A. 1. and VI. B. 1. of the <br />rationale, the permittee must submit analyses which demonstrate compliance with the "No Discharge" <br />limitation specified in section VI.A. 1. of this rationale. Such demonstration may incorporate whatever <br />analyses the permittee determines is necessary, but the annual report must include the speck <br />analyses listed under the next heading. <br />(2J For each ojrhe numbered discharge points specfcally identified in the tables of effluent limitations <br />and monitoring conditions listed in sections VI. A. 1. and Vl.B.1. of the rationale, the permittee must <br />submit the following analyses of (a) ows, (bJ pollutant concentrations, and (cJ pollutant mars <br />loadings measured at or calculated for each point: <br />iJ n statistical sumnmry of the data collected during the year for which the report is being prepared; <br />iiJ a comparison of the most recent year's data with pre-project data; <br />iiiJ trend mmlyses showing how flows, pollutant cm~centrations, and pollutant loadings might be <br />changing over time; <br />itJ a summary of the most recent year's pollutant concentrations which exceeded stream standards. <br />(3J a summary of all discharges from sources not specifically identhed in the permit, or discharges from <br />the Valley Leach Facility that qualified for the annual precipitation or storm exemptions listed in <br />section VI. A. 2. and 3. of tftis rationale. The summary for each discharge shall include: <br />iJ the location of the discharge; <br />iiJ the dates when the discharge occurred; <br />iiiJ a briejdircussion of how the discharge occurred and how it was or will be eliminated; <br />ivJ a statistical summary of the discharge flows, pollutant concentrations, and pollutant loadings; <br />
