Which action to follow is dependent upon how much <br />leakage is found. Using the criteria and corresponding responses specified in the DMG permit application, the <br />permiuee shall determine which of the following actions is appropriate, shall perform the indicated action, and <br />shall use the action codes that are listed to calculate values that must then be reponed on the monthly Discharge <br />Monitoring Reports (DMR,cJ. <br />I Normal operation continuing -collected leachate must 6e pumped back to the solution circulation system <br />2 The leakage monitoring frequenry must be increased to at least twice the frequency listed in Table V/-7, and <br />any observable dmnage to the liner must be repaired. <br />3 Water management must be improved in order to reduce the leakage rate. Special not cation must be <br />provided to the Water Qualiry Control Division and the Division ojMinerais nrul Geology indicating what the <br />leakage rate is and x~hnt measures have been taken ro reduce it. The permiaee shall provide verbal <br />not cation the WQCD within 24 hrs, and written notification within 5 dm~s. <br />4 Additional measures mast be implemented in order to reduce leakage, which may include the inspection and <br />repair ojthe liner or the reduction of head over the liner. Special notification must 6e provided to the Water <br />Quality Control Division and the Division ojMinerair and Geology indicating what the leakage rate is and <br />what measures have been taken to reduce it. The permiaee shall provide verbal notification to the WQCD <br />within 24 hrs, and wrinen notification within 5 days. <br />In addition to [he nottfication provisions listed above, when cyanide is detected for points 002A or 003A, the perminee <br />shall provide verbal notification to the WQCD within 24 hrs, and written notification within S days. <br />
