Cattany <br />Division Director <br />Natural Resource Trustee <br />Listed below are adequacy review comments on the River.Pazk Recreation Access New 110 <br />Application, Permit Number M-2005-051. The Division of Minerals and Geology (Division) is <br />required to make an approval or denial decision no later than August 11, 2005, therefore, your <br />response to the following adequacy review concerns should be submitted to the Division as soon <br />as possible. <br />1. As required by Rule 1.6.2 (1)(g), please submit the proof of publication of the public notice <br />circulated in the local newspaper, all owners of record of surface Lands, mineral rights, <br />easement holders, and owners of record of surface lands that are within 200 feet o1;_the <br />boundary of the affected land. Proof of notice maybe return receipts of a Certified Mailing <br />or by proof of personal service. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional <br />engineer, or other qualifis~i person. The maps submitted to the Division were not signed. <br />Please resubmit a signed copy of the maps. <br />Rule 6.3.2 Exhibit B- Site Description <br />The applicant states that they have meet with the Core•of Engineers on site to determine if <br />wetlands exist. Please inform the Division if wetlands exist on site, and locate the area on <br />the exhibits. In addition, please inform the Division if a 404 Permit will be obtained. If <br />wetlands aze present, please state if the area is to return as a wetland area and provide <br />additional information regazding the reclamation of the wetland. In particulaz, please state <br />the fmal reclaimed slopes, depth of topsoil to be replaced and the seed mixture to be utilized. <br />4. The applicant proposes to add an additional 100 feet of pipe to the existing pipeline to <br />facilitate the access road design.
