TETRATECH RMC Page 4 <br />Mr. Berhan Keffelew <br />July 27, 2004 <br />B) The operator agrees to establish baseline water levels in all wells with in 600 <br />feet of the permit area at lease once prior to mining. <br />The operator has agreed to do this. <br />C) Operator agrees to monitor monthly, all monitoring wells until mining begins <br />and quarterly after mining begins and until reclamation is completed and the <br />site is released from reclamation warranty. The quarterly report to be <br />submitted to DMG, on a semi annual basis. <br />The operator requests that they be allowed to submit the quarterly monitoring <br />with the annual report rather than semiannually. Please let us know if this is <br />acceptable. <br />D) During the pre-operational inspection, we agreed that the operator will <br />provide a ground water model, to show the probable impacts to the deep wells <br />and the aquifer, that are being used as domestic wells. Operator also agrees <br />to mitigate all well owners with in 6000 feet of the permit area, if cause can <br />be shown that the operation did impact the wells. The ground water model <br />must include probable impacts to Fulton Ditch especially during the mining <br />operation along the East side. <br />The operator agrees to mitigate all wells within 600 feet of the permit area, <br />not 6000 feet as stated in your comment. The groundwater model is attached <br />for your review. <br />VII) EXHIBITS "H', "I", "J"AND "K'ADEQUATE <br />Noted <br />VIII) EXHIBIT "L "RECLAMATION COST <br />A) In your reclamation cost, you did not include the warranty needed to remove <br />the conveyer system under 104`h ave. In order to calculate the warranty, the <br />Division needs the dimensions for the systems and associated parts, With out <br />including the conveyer system, the Division has calculated a financial <br />warranty in the amount of $398,871. The detailed calculation is enclosed with <br />the adequacy. <br />The conveyor system is portable and an asset of the applicant. Please refer to <br />item III-A above. <br />
