' State Highway Access Permit Pa9E <br />• Attachment to Permit No. 105070 - AddlUonal Terms and Conditions <br />Personal protective equipment (e.g. head protection, footwear, high visibility ap are1, safety glasses, hearing <br />protection, respirafnrs, gloves, etc.) will be warn as appropriate for the work bi g performed, and as specified <br />in regulation. At a minimum, all workers in the State Highwayright-of-way, ex apt when in their vehicles, <br />will wear the following personal protxtive equipment: <br />1. Head protection that complies with the ANSI Z89. t-1997 standard; <br />2. At all construction sites or whenever there is danger of injury to feet, worker will comply with O$IlA'S <br />PPE requirements for foot protection per 29 CFR i9t0.136, !926.95, end 1926. 6. If required, such footwear <br />will meet the requirements of ANSI 7A1-1999; <br />3. High visibility apparel as specified in the Traffic Control provisions of this p 't (at a minimum, <br />ANSUSEA 107-1999, Class 2). <br />Where any of the above-referenced ANSI standards have been revised, the mos recent version of the standard <br />will apply. <br />29. Any damage to any present highway facilities including traffic control devices s all be repaired immediately <br />at no cost to the Department or issuing authority and prior to continuing other rk. Any mud or other <br />material tracked or otherwise deposited on the roadway shall be removed daily r as ordered by the <br />Department or issuing authority inspector. <br />30. Areas of roadway and right of-way disturbed during this installation shall be r ored to their original <br />condition to insure proper strength, drainage and erosion control. <br />31. If petroleum or other potentially hazardous material is encountered during axes lion ,the work shall be <br /> stopped immediately and the Department notified at 303.757-9126. The prop disposal of any soil ar other <br /> material determined to be hazardous and/or contaimnated either uncovered or a cavated during construction <br /> shall be the responsibility of the Petntittee. The disposal shall be accomplished " accordance with all <br /> applicable federal, state, and local laws end regulations and at no cost to the De artment or issuing authority. <br />32. All areas of the State Highway right-of-way disturbed during construction shall a covered with a minimum of <br /> 4 inches of top soil end re-seeded according to the Department's specifications r grass seed mix. <br />33. When it is necessary to remove any highway right-of-way fence, the posts on ei er side of the access entrance <br /> shall be securely braced with approved 'end posts and in confottnattce with the tparunent's M-607-1 <br /> standard, before the fence is cut, to prevent slacking of the remaining fence <br />34. The Permittee shall install a right rum deceleration lane and a right turn accei lion lane on State Highway <br /> 119, in conformatce with section 4 of the State Highway Access Code, 2CCR 6 1-1, and the approved <br /> consruction plans. <br />35. The Permittee shall install a left turn acceleration and left turn deceleration lan on State Highway 1 ] 9, in <br /> conformance with secton 4 of the State Highway Access Code, 2CCR 601-1, d the approved construction <br /> plans. <br />36. All work that requires traffic control shall be supervised by a registered professi nal traffic engineer or by a <br /> traffic control supervisor certified by the American Traffic Safety Services Ass ciaton (ATSSA) or the <br /> Colorado Contractors Association (CCA). When flagging personnel are requir ,they shall be certified by the <br /> contractor in accordance with the Department standards. <br />• 37.
