State Highway Access Permit p <br />• Attachment to Permit No. 105070 -Additional Terms and Conditions <br />PermitteelContractorlEngineer shall submit an "As Built" plan, showing in data 1 all construciton changes, <br />modifications and revisions. All changes, modifications and revisions shell be eared and signed by a <br />Colorado Registered Professional Engineer. The submittals shall be placed in box labeled with all its <br />contents and shall be submitted tothe Departrnent representative. <br />24. The Department will require the permittee to retain a Professional Engineer (P to be in Responsible Charge <br />of Construction Observation. The PE's responsibilities include, but are not limi ed to: <br />1. The PE in Responsible Charge of Construction Observation shall evaluate c mpliance with plans and <br />specifications with regard to the roadway improvements within the State High y Right of W ay . The PE's <br />responsibilities shall be as defined in Section 5 of the BYLAWS AND RULES F THE STATE BOARA OF <br />REGISTRATION FOR PROFESSIONAL ENGINEERS AND LAND SURVE ORS and the relevant sections <br />of the latest CDOT Standard Specifications for Road and Bridge Constructiott. he PE shall carefully monitor <br />the contractor's compliance on all aspects of construction including Constructio Work Zone Traffic Control. <br />2. Engineering Certification: After inspection and before conditional acceptan e, the PE shall certify to <br />CDOT, in writing, that all inspecfions, materials, materials testing, and constm Lion methods conform to the <br />plans, specifications an~ purpose of the design. The PE shall be experienced s d competent in road and <br />bridge construction management and inspection and materials testing. Certific 'on is defined as a statement <br />that includes all of the following: <br />a. Is sealed and signed by a PE representing that the engineering services addr ed therein have been <br />performed by the PE, or under the Professional Engineer In Responsible Charg ; <br />. b. Is based upon the PE's knowledge, information, and belief; and <br />c. Ts in accordance with all applicable CDOT standards and the approved cons coon plans. <br />The letter of certificaiton end a copy of all test results shall be sent to The Colo ado Department of <br />Transportation, Region One Traffic Section at 18500 East Colfax Ave., Aurora CO. 800! 1. <br />2S. The Permittee must have available for review at the time of conditional accept ce, all documents relating to <br />construction (ie: plans, project correspondence, materials testing and survey no s). <br />26. In accordance with Section 2.5 (~ of the State Highway Access Code, final ace prance of the access <br />improvements by CDOT, Region One, will be provided to the Permittee in wri g two (2) years from the date <br />of construction completion. If you have not received a final acceptance letter i two years, please contact the <br />Region One Acceas Manager at 303-757-9126. <br />27. It shall be the responsibility of the Permitee to verify the location of the existin utilities and notify all utility <br />owners or operators of any work that might involve utilities within the State Hi way right-of--way. Any work <br />necessary to protect existing pernvtted utilities, such as an encasement will bet a responsibility of the <br />Permittee. Any damage or disruption to any utilities during the construction sh 11 be the Permittee's <br />responsibility and shall be repaired or replaced at no oast to the Deparbnent, th issuing authority, or the local <br />jurisdiction. Utilities cannot remain under asphalt or concrete without prior ap royal from CDOT. <br />28. A11 workers within the State Highway right-of--way will comply with their empl yer's safety and health <br />policieslprocedares, and all applicable U.S. Occupational Safety and Health A inistration (OSHA) <br />regulations -including, but not limited to the applicable sections of 29 CFR Pa 1910 -Occupational Safety <br />• and Health Standards and 29 CRF Part 1926 -Safety and Health Regulations fo' Construction. <br />
