<br /> <br />Mr. Thames Hartley -3- July 12. 1995 <br />The Division will further require that field trial seams be prepared <br />and destructively tested at the beginning of each seaming period, <br />and at least once each four hours, for each seaming apparatus used <br />that day. Destructive samples must be randomly cut and tested <br />during liner installation, at an average frequency of one test for <br />each 500 feet of welded seam. Destructive testing to be in <br />accordance with test method ASTM D 4437. <br />Obviously, hot shoe double welding cannot be used for detail or <br />repair work. The Division will require that conventional extrusion <br />welding be used for all areas that cannot be hot shoe welded, <br />including the underdrain pipe boot location. All extrusion welds <br />will have to be tested for leaks. <br />9. The Division shares the Committee's concern with the potential for <br />wind damage to the liner during installation. Please commit to <br />having a sufficient number of sand bags on hand to provide <br />uninterrupted coverage for any un-anchored liner edge at any point <br />during installation. <br />10. The Committee's comments regarding underdrain installation are well <br />taken and will have to be addressed by COM, Inc. In particular, a) <br />specify the elope of the solid section oP the underdrain pipe, <br />between the connection to the perforated section and the proposed <br />valve box; b) specify the minimum thickness of hand compacted fill <br />that will be placed on the solid section of underdrain pipe prior to <br />allowing mechanized compaction to resume, and; c) commit to <br />establishment of a minimum 2 percent grade for all perforated <br />sections BP underdrain page. A1_sQ, the suggestion that a reservoir <br />be installed at the underdrain pump is a good one, in that <br />maintaining a head on the pump will prevent cavitation. <br />COM, Inc. is under a Board Order to initiate mill site reclamation on July <br />28, 1995. The obvious course to take in any effort to have this Board <br />Order reversed, would be to gain approval of the permit application, and <br />issuance of the permit. In order for the Division to approve the <br />application, COM, Inc. must adequately address the issues raised in this <br />letter, as well as those issues enumerated in the Division's letters dated <br />June 30, 1995, and June 25, 1995, and complete the on the ground work <br />required in the Division's 2/28/95 and 6/7/95 inspection reports. The <br />required on the ground work includes establishment of the interim storm <br />water retention berm on the tailing dam crest, maintenance of upland <br />diversion ditches, removal of tailing from the Hazel-A adit, repair of <br />decant risers so that they extend above the level of the tailing surface, <br />and repair of all liner defects and tears. Expedience in addressing these <br />issues and completing this work is clearly called for. <br />
