State Highway Access Permit Page z <br />Attachment to Permit No. 103039 -Additional Terms and Conditions <br />highway BEFORE 3:30 pm each day. <br />12. No work within the roadway and the State Highway Right-of--Way is allowed at night, on Saturdays, Sundays, <br />on holidays, during adverse weather conditions, or between November first and April fifteenth of each yeaz <br />without prior approval from the Department. <br />13. A FULLY EXECUTED COMPLETE COPY OF THIS PERMIT MUST BE ON THE JOB SITE WITH THE <br />CONTRACTOR AT ALL TIMES DURING THE CONSTRUCTION. FAILURE TO COMPLY WITH THIS <br />OR ANY OTHER CONSTRUCTION REQUIItEMENTS MAY RESULT IN THE IMMEDIATE <br />SUSPENSION OF WORK BY ORDER OF THE DEPARTMENT INSPECTOR OR THE ISSUING <br />AUTHORITY. <br />14. 'During Construction, the Permittee or the contractor shall procure and maintain general public and auto <br />liability, and property damage insurance covering the operations under this permit in the amounts specified in <br />seciton 23-10-114, C.R.S.. Policies shall name the State of Colorado as an additional insured party. Like <br />coverage shall be furnished by or on behalf any subcontractor (s). Certificates of insurance showing <br />compliance with these provisions shall be attached to and made part of this permit and be available on the site <br />during construction. <br />15. The State requires a Certificate of Insurance prior to commencing any work on the highway right-of--way. All <br />vendors, contractors, and utility companies shall procure, at their own expense, and maintain for the duration <br />of the work period, the following minimum insurance coverage: <br />A. Standard workman's compensation and employer's liability, including occupational disease, covering all. <br />employees engaged in performance of the work at the site, in the amounts required by State Statues. <br />B. Comprehensive general liability in the amount of $600,000.00 combined single limit bodily injury and <br />property damage, each occurrence and $2,000,000.00 annual aggregate. <br />C. Automobile liability in the amount of $600,000.00 combined single limit bodily injury and properly <br />damage, for each accident. <br />16. All materials, equipment, installation, construction and design, including the auxiliary lane (s) and intersection <br />improvement (s) within the State Highway shall be in accordance with the following Department standard <br />references, as applicable: <br />a. Roadway Design Manual <br />b. Materials Manual <br />c. Construction Manual <br />d. Standard Specifications for Road and Bridge Construction, latest ediiton <br />e. Colorado Standazd Plans (M & S Standards) <br />f Manual On Uniform Traffic Control Devices (MUTCD) for streets and highways and the latest Colorado <br />Supplements <br />g. A Policy on Geometric Design of Highways and Streets, American Association of State Highway and <br />Transportation Officials (AASHTO), latest edition <br />h. Institute of Transportation Engineer's Trip Generation manual, 6th edition <br />i. State Highway Access Code (2 CCR 601-1) <br />j.
