C. Pre-Mining & Miniag Plan Maps -Per Rule 6.4.3(g), the Applicant will reseazch the physical <br />location of the Silver Dollar Metro District light poles and revise the pemtit maps, as necessary, to show <br />the location of the light poles if they occur within 200 feet of the affected land of the MMRR Quarry <br />operation. (Affected land includes the quarry access road off Stato Highway 119). Please clarify whether <br />the light poles occur within 200 feet of the proposed affected ]and under either scenario "B" or "C". <br />Please respond. <br />D. Exhibit S - If the Silver Dollaz Metro District light poles, as noted in Item C above, occur within 200 <br />feet of the proposed affected land with access point "C", the Applicant will need to demonstrate <br />compliance with Rule 6.4.19. Please respond. <br />E. Exhibit A -Legal Description :Per Rule 6.4.1 the Applicant will need to provide the correct latitude <br />and longitudinal coordinates of access point "C". Please provide. <br />F. Application Form -Per Rule 1.4.1 (1), please provide a revised Regulaz 112 Application form which <br />cleazly lists the permitted and affected land acreage to be permitted. Attached aze two copies of a Regulaz <br />112 Application Form. Also, please clarify the changes in permitted acreage and affected land acreage <br />under access points "A", "B", and "C". Please provide. <br />G. City of Black Hawk & Gilpin County Comments -The Division affirms its previous request on <br />September 16, 2005 for the Applicant to provide a written response to September 12, 2005 correspondence <br />the Division received from the City of Black Hawk via Hayes, Phillips, Hoffman & Cazbeny, and the <br />County of Gilpin via Petrock & Fendel. Please provide. <br />Once again, we will look forwazd to your response to Division adequacy concerns on or before October 31, <br />2005 to allow adequate time for review before the November 15, 2005 recommended decision date. Please <br />be sure to submit two (2) copies of all documents. Rule 1.8.1 states that changes and additions to the <br />permit application must be filed with the Division and the County Clerk and Recorder.
