Div. 4 W. Dist, 40 <br />Dear Mr. Farnsworth: <br />Enclosed is one copy of the recently completed Phase I Inspection <br />Report on the above referenced dam prepared by Hydro-Triad, Ltd. These <br />inspections were performed in accordance with the U. S. Government <br />Contract No. DACW 45-78-C-0023, with the State of Colorado under the <br />Public Law 92-367, the National Dam Safety Act. We request that you <br />thoroughly review the report and implement the actions recommended therein, <br />in order to increase the safety and operability of your structure. Engineers <br />from our Dam Safety Branch will be available to discuss any questions you <br />may have regarding the report. <br />Based on this Phase I inspection, a review of our file and inspections <br />by our field engineer, we consider this dam to be inadequate for full storage. <br />Due to the inadequacy of the spillway to discharge the high hazard design <br />storm and the relatively high exit point of seepage on the downstream slope, <br />storage behind this structure is hereby restricted to a water level 10 feet <br />below the lowest point in the crest of the dam. By copy of this letter, the <br />Division Engineer is directed to enforw this restriction. <br />If you wish.to use the full storage capacity of the reservoir in the <br />future, you must have plans and specifications for the repair of the dam and <br />construction of an adequate spillway prepared by an engineer registered in <br />the State of Colorado and submit them to this office for approval. Upon approval <br />of the plans and specifications and completion of repair work, a final inspection <br />will be made by this office. After the repairwork has been inspected and found <br />. satisfactory, the storage restriction will be removed. <br />
