<br /> <br />Adequacy Issues <br />Stone Gravel Pit <br />4 <br /> <br />December 17, 1997 <br />Permit No. M-97-089 <br />ensure no unauthorized release of pollutants to groundwater, Rule 3.1.5(11), by <br />ensuring that all fuels, lubricants and chemicals, stored and utilized within the permit <br />area, are stored in a controlled manner at a designated location. Fluid and chemical <br />storage location should be surrounded by spill containment berm and lined with <br />impermeable liner to ensure that spilt fluids and/or chemicals not escape. <br />8. Reclamation Plan Map -Contour Interval -Rule 6.4.6(a) requires that the map show <br />proposed topography of the area with contour lines of sufficient detail to portray the <br />direction and rate of slope of all reclaimed lands. <br />Reclamation Plan Map shows the 5840' contour line, located downslope of the mining <br />limits line, and the 5880' contour line, interrupted by mining activities and presumably <br />incorporated within the 3H:1 V backfilled slope line. However, direction and rate of slope is <br />not indicated within reclaimed areas except for the 3H:1 V backfilled slope. <br />Pursuant to Rule 6.4.6{a), please provide a Reclamation Plan Map, with contour interval of <br />less than 40', as necessary to portray the proposed direction and rate of slope for all <br />reclaimed areas. <br />ADDITIONAL ISSUES <br />Other Permits - Exhibit M of the application indicates that applicant holds or will be seeking <br />the following permits: <br />Stormwater Permit from WQCD -CDPHE; <br />Equipment Emissions Permit from APCD -CDPHE; <br />APEN Permit from APCD -CDPHE; and <br />Road Approach from Montezuma County Roads Department. <br /> <br />DMG has received copy of WQCD -CDPHE letter, dated 11/17/97, indicating that WQCD <br />has not received application for the required Stormwater Permit. <br />Issuance of permits, required from other agencies, is PQt a condition for DMG permit <br />approval.
