Several sections of the Board's Regulations require the application to contain information <br />pertaining to distances to certain items from the permit azea, the blasting azea, the affected <br />area, etc. Please provide the following information. If some or all of this information <br />is already contained in the application, please discuss where it may be located for <br />continued review. <br />a) Please provide a map that indicates the on-the-ground boundary limits of the <br />blasting azeas of the surface mine (Sections 4.08.3(2)(b)(i), 4.08.4(3), 4.08.4(4), <br />4.08.4(7), 4.08.4(8), and 4.08.4(10)). <br />b) On this same map, please indicate the location of the dwelling or other occupied <br />structure nearest the blasting area limit, and indicate the point that lies halfway <br />between this structure and the blasting area limit. Identify this halfway point as <br />the "fly rock travel limit" (Section 4.08.4(8)). <br />c) On this same map, please indicate the boundary of the area to which access by <br />unauthorized personnel and livestock will be regulated and controlled (Sections <br />4.08.4(5) and 4.08.4(8)). Designate this the "regulated access area". Please note <br />that, pursuant to Section 4.08.4(8), if the "regulated access area" limit is <br />designated to be closer to the blasting azea limit than the point halfway between <br />the blasting azea limit and the nearest occupied swcture, the '4egulated access <br />area" limit will become the "fly rock travel limit", as fly rock may not travel <br />beyond the "regulated access area" limit. <br />d) Please identify on a map the location of all dwellings, public buildings, schools, <br />churches, community buildings, and institution buildings within one mile of the <br />blasting area limit. Please identify the permit area boundary on this same map <br />(Section 4.08.4(10)). This will assist the Division in determining what maximum <br />allowable peak particle velocities for ground vibrations may apply to certain <br />structures in the vicinity of the mine. <br />e) [n addition to the information provided above, please also provide the location of <br />any hospitals or nursing facilities within 1000 feet of the blasting area limit <br />(Section 4.08.4(7)(a)), and the locations of structures such as, but not limited to, <br />those described in Section 4.08.4(7)(b) that lie within 500 feet of the blasting area <br />limit. <br />f) If not already done so by the information requested above, indicate the locations <br />of all structures (roads, irrigation ditches, wells, head gates, power poles, <br />underground cables, pipelines, livestock tanks, culverts, sheds, barns, garages, and <br />rail lines) that lie within one half mile of the blasting area limit (Section 4.08.2(1) <br />and 4.08.4(4)). This will assist in the Division in determining which entities may <br />be entitled [o notices regarding pre-blast surveys, blasting schedules, and blasting. <br />
