Inspection Contact <br />Page 3 of the application form has been revised to designate an inspection contact <br />and redesignate [he permit contact. <br />Tab 3, page 1 has been updated to include the names and addresses of both the <br />owner/applicant and operator, including a local office address and telephone number. <br />8. MSHA Permit Number <br />Tab 3, pages 7 and 9 have been updated to include the facility MSHA identification <br />number. <br />9. Sales Tax Number <br />Tab 3, page 10 has been revised to delete the pending sales tax license which is not <br />needed by the owner/applicant. <br />•The Licenses end Permits section of Tab 3 (pages 9 and 10) was revised to add a <br />rail crossing license from the Department of Transportation, add the CPDES Permit <br />expiration date, and correct the air quality permit numbers and expiration dates. <br />10.
