Mr. Dan Mathews, MLRD <br />Page 3 <br />Rule 2.03.9 Insurance Certificate <br />Terror Creek Company has personal injury and property damage insurance <br />under policy numbers 583JM3443 and 183J88634 as issued by the St. Paul <br />Fire and Marine Insurance Company through their agent, McCaskill-King <br />Insurance Service, Boulder, Colorado. The effective dates of the subject <br />policies was April 26, 1983. <br />Because of the limited time involved, a certificate of insurance form <br />was not available; however, copies of the pertinant pages from the actual <br />policies which, according to our agent, should be sufficient to prove <br />existence of the necessary insurance. <br />In addition, Terror Creek Company employees are covered under Workmens <br />Compensation Insurance as issued by the State Compensation Insurance Fund, <br />policy number 148692-2. A copy of the coverage notice letter is enclosed. <br />I believe this letter and accompanying enclosures will be acceptable <br />in response to the adequacy comments specified.
