The insurance policy must include a rider requiring that the insuror <br />notify the Division of any substantive changes in the policy or failure <br />to renew same. The certificate of insurance submitted by the applicant <br />states that an endeavor will be made to notify the Division instead of <br />a specific requirement. This must be changed and proof of the change sent <br />to the Division. <br />2.04.4 Cultural and Historic Resource Information <br />The following is a summary of the deficiencies in the cultural and historic <br />resource information. The complete Office of Surface Mining review is <br />attached for further information: <br />1. A detailed map of areas surveyed for cultural .resources is needed. <br />On the map, areas of present and proposed surface disturbance should be <br />plotted, and the boundaries of the "study area" defined. was the survey <br />of Coal Gulch and the Coal Canyon road opportunistic, random or total <br />coverage? How many rock shelters were visited and what is the potential <br />for finding more? <br />2. A general cultural resource background overview of the mine plan area <br />is needed. It should include a discussion of both the history and <br />prehistory of the area. Other cultural resource surveys in the area <br />should be referenced. <br />3. Amore detailed description of the survey areas (acres, visibility, <br />etc.) and methodologies employed during the survey is needed. <br />4. Is the submission in the mine plan the complete WignalI report? Has <br />the report been edited for the submission? If so, provide the original <br />data as some deficiencies may be corrected by this report. What is <br />the date of the Wignall report? Where are Plates I, II, III, Iv, and <br />VII? These must be submitted. <br />5. Further explanation of the nature and extent of the "trowel tests" <br />conducted under boulders are needed. <br />6. A sample survey of potential subsidence areas needs to be done. The <br />noted presence of rockshelters makes this survey all the more <br />important. The survey is to identify sensitive sites (rock art, rock <br />shelters, historic and public structures) which may be impacted by <br />post mining subsidence. What is the predicted subsidence? <br />7. The National Register check needs to be documented. <br />8. Are any of the early mine workings still present? rf so, these <br />should be recorded if they are historic sites. <br />
