The applicant must submit the names and addresses of the holders of <br />record of any leasehold interest in the property to be affected by <br />surface operations and facilities, if there are any. Otherwise, the <br />applicant must state that there are none. <br />2. The applicant must submit the names and addresses of any purchaser of <br />record under a real estate contract of the coal to be mined. If there <br />are none, this must be stated. <br />3. The application must contain the names and addresses of the principals, <br />officers, and resident agents of the Cameo Land Company. <br />4. There are several discrepancies between the tables showing surface <br />and subsurface ownership within and contiguous to the permit areas and <br />exhibits 3 and 4 showing ownership. The following is a list of the <br />discrepancies: <br />a. James Brothers Coal Company, Powder Mountain Ranch, H. Hays and <br />Albertson Ranch are shown as surface owners on Exhibit 3, but the <br />names and addresses and not listed in the text. <br />b. The town of Palisade is listed as a surface owner in the text, but <br />is not shown on Exhibit 3. <br />c. Gay Johnson, H. Hays and Grand Valley Water Users Association <br />are shown as subsurface owners in Exhibit 4, but are not listed in <br />the text. <br />d. Palisade Coal and Supply and Charlie Viomar are listed as subsurface <br />owners in the text, but the areas they own are not shown on Exhibit 4. <br />e. George Nronzy should be listed as a surface and subsurface owner <br />in Table 2.03.4 c. <br />5. The applicant should explain why the No. 2 east mains (B-section) of <br />the Roadside A9ine and the north mains of the Cameo No. 1 mine have the <br />same MSHA identification numbers. Is this a typo? <br />2.03.8 Permit Term Information <br />1. The application must state the anticipated starting date and termination <br />date of each phase of the underground mining activities and the anticipated <br />number of acres of surface Lands to be affected during each phase and the <br />horizontal and vertical extent of the proposed underground workings over <br />the anticipated life of the mine. <br />
