The applicant should list permit application C-036-81 <br />in their response to Section 2.03.4(4). <br />2. The applicant should submit the MSHA identification <br />number as required by Rule 2.03.4(6) when it has been received. <br />3. The applicant should submit the statement of all <br />lands, interests in lands, options, or pending bids on <br />interests held or made by the applicant for lands which are <br />contiguous to the permit area as required by Rule 2.03.4(7). <br />This information may be held as confidential by the Division. <br />2.03.10 - Identification of Other Licenses and Permits <br />1. The applicant should submit an indication that the <br />Colorado Department of Health, Air Pollution Control Division, <br />has been consulted about the need for additional emission <br />permits. <br />2. The current NPDES permit will need to be revised. A <br />copy of the new permit should be submitted. <br />2.03.12 - Proof of Publication <br />1. The applicant should submit proof of publication as <br />soon as it is available. <br />2.04.4 - Cultural and Historic Resources <br />1. The applicant has conducted an independent surveyof <br />the permit revision area as indicated on page 2.04-4 of the <br />permit application. This study has not yet been submitted to <br />the Division. Preliminary adequacy review of this section <br />cannot commence until these materials are received.
