<br />2.03.9 Personal Injury and Property Damage Insurance <br />Information <br />(1) A certified copy of insurance for personal <br />injury and property damage protection for an <br />adequate amount to compensate any person <br />injured or property damaged as a result of <br />surface or sub-surface coal mining and recla- <br />mation operations, including use of explozives. <br />Minimum insurance coverage for bodily injury <br />shall be $300,000 for each occurrence and <br />$500,000 aggregate; and minimum insurance <br />coverage for property damage shall be $300,00 <br />for each occurrence and $500,00 aggregate. <br />(2) The policy will be maintained in full force <br />during the life of the permit and any renewal <br />thereof, including completion of all recla- <br />mation operations under Rule 4. <br />(3) The policy will contain a rider requiring that <br />the THE BLUE FLAME COAL COMPANY will notify the <br />Divi~~.,,: whenever substative changes are•made in <br />the policy, including any termination or failure <br />to renew. <br />(4) The BLUE FLAME COAL COMPANY, if acceptable, shall <br />submit satisfactory evidence of self-insurance <br />capabilities to the Department, if and when Blue <br />Flame would elect the method of self-insurability. <br />2.03.10 Identification of Other Licenses and Permits <br />(1)(a) Colorado Department of Health, Water Quality <br />control Division, National Pollutant Discharge <br />Elimination System (NPDES) Permit. <br />(b) Colorado Department of Health, Air Pollution <br />Control Division, Emission Permit. <br />(c) United States Environmental Protection A~ency, <br />Prevention of Significant Deterioration PSD) <br />Review. <br />(d) Colorado Division of Mines, Underground Mining <br />Permit. <br />(2) Applications and approvals pending. <br />
