Tate -2- February 17, 1982 <br />From review of the applications filed by the Snowmass Coal Company, Grand <br />Mesa Coal Company, Powderhorn Coal Company and the Qlue Ribbon Coal Company, <br />it appears that all of these companies are under the common control of these <br />same partnerships. <br />As required by 2.03.9(2)(b), the applicant must provide the names and addresses <br />of any person who is a principal shareholder in the North Fork Energy <br />Corporation or Nawk Mining, Inc. <br />As required by 2.03.4(4), all current of previous coal mining operations held <br />by any persons identified in 2.03.4(2)(b), or the applicant, must be identified. <br />As required by 2.03.5(1), a statement of whether the applicant, any subsidiary, <br />affiliate or persons controlled by or under common control with tl~e applicant <br />has: <br />(a) Had a Federal or State mining permit for surface coal mining operations <br />suspended or revoY.ed in the last five years prior to the date of submission <br />of the application; or <br />(b) Forfeited a mining bond or similar security deposited in lieu of bond. <br />2. if any such suspension, revocation or forfeiture has occurred, a statement <br />of the facts involved, including: <br />(a) Identification number and date of issuance of the permit or date and <br />amount of bond or similar security; <br />(b) Identification of the authority that suspended or revoked a permit <br />or forfeited a bond and the stated reasons for that action; <br />(c) The current status of the permit bond, or similar security involved; <br />(d) The date, location, and type of any administrative or judicial <br />proceedings initiated concerning the suspension, revocation or forfeiture; and <br />(e) The current status of these proceedings. <br />3. A schedule listing any and all notices of violations of the Act and any <br />applicable law of the United States or of this State, or any applicable rule <br />or regulation of any department or agency of the United States, other states, <br />and this State, pertaining to air or water environmental protection received <br />by the applicant in connection with any surface coal mining operations during <br />the three-year period prior to the date of application. The application schedule <br />shall also include a statement regarding each violation notice, including: <br />(a) The date of issuance and identity of the issuing regulatory authority, <br />department or agency; <br />
