Dale Larsen -2- September 11, 1984 <br />5) Rule 2.07.3(2) requires that "at the time of submission of an <br />application for a permit the applicant shall submit to the Division the <br />proposed notice of publication..." That notice was not submitted. <br />6) Rule 2.04.4 - Cultural and Historic Resource Information, requires the <br />filing of a statement from the State Department of Historical Preservation. <br />This statement has not been provided in the application. <br />7) Rule 2.04.7(2)(b) requires surface water information which includes <br />flows which identify critical low flow and peak discharge rates, and water <br />quality data from surface waters in, discharging into, or which will receive <br />flows from surface or ground water from affected areas within the proposed <br />area, sufficient to identify seasonal variations. The application does not <br />include adequate surface water information to make judgments on seasonal flow <br />or water quality variations. <br />With the receipt of the above information, the Division will continue its <br />review of your application. If you have any questions, please contact us. <br />8) Rule 2.05.4(2)(b) requires a detailed estimate of the cost of <br />reclamation of the proposed operations required to be covered by a performance <br />bond with supporting calculations for the estimates.
