1 ' <br />2.05 APPLICATION FOR PERFIIT FOR SURFACE OR UNDERGROUND MINING ACTIVITIES <br />P17NIMUh1 REQUIREh1ENTS FOR OPERATION AND RECLAMATION PLAN <br />2.05.3 Operation Plan - Permit Area <br />1. The applicant should clarify the meaning of the label "Room workings" <br />on Map IV-4. Are these previously mined areas, or areas to be roomed and <br />pillared? Also, explain the difference in methodology between "Pillar" <br />and "Pull" in Table IV-1, page IV-26 and IV-27. <br />2. The applicant has stated on page Iv-25 that "the approval of this <br />permit (for both mines) represents an activity permit within the permitted <br />acreage and is not bounded by a maximum production". It should be noted, <br />the Division approval of any permit application only represents approval <br />of those activities described in the application. If those activities <br />are to be modified as a result of increased production, the applicant <br />must submit a plan of the modified activities in the form of revisions <br />to the permits for Division review. <br />3. Sections named SS, 9N, ION, 11N, 12N and Z3N need to be identified <br />on the mine map for the b5 mine. Also, on Map Iv-9 the northwest <br />corner, please explain the meaning of panels "extending to rock". <br />4. If mine water from the b5 mine continues to be pumped into the N4 <br />mine workings, the Division cannot accept the statement on page Iv-2 <br />disregarding responsibility for this mine. If a previously disturbed <br />area is redisturbed, the company must comply with the appropriate performance <br />standards. <br />2.05.3(3) Mine Facilities <br />1. The description of Electric Substations on page IV-ZI is missing part <br />of the narrative; this should be corrected. <br />2. The main office and engineering office were relocated to Empire <br />Village in 1980 according to the list on page IV-10. On Page IV-11 the <br />locations in Empire village are identified as temporary. Will these two <br />facilities be relocated? If so, please discuss when and where. <br />3. The location of the 10,000 ton storage silo is located within 100 feet <br />of a perennial stream. Permit N79-190 was granted for this structure <br />in 1979 with several stipulations. For the purpose of this permit review <br />the Division must make a written finding to grant approval for the variance <br />from the 100 foot stream buffer zone (Rule 4.05.18). The applicant <br />should address this variance and include a discussion of the bank <br />stabilization of the Williams Fork undertaken by Gmpire and t/[e stream <br />monitoring program. <br />2.05.3(3)(c) Roads <br />1. A description of each road, conveyor or rail system to be constructed, <br />used or maintained within the proposed permit area must be supplied. The <br />applicant should include a map with this description clearly delineating <br />each haul, access and light use road proposed to be used during the term <br />of the permit. <br />-6- <br />
