Mitch ltollings of the <br />09ice of Surface Mining conducted oversight on the iospective. <br />Most of the site was dry, but these had been heavy precipitative in the woek prior to the iospec6ve. <br />$~ were meted, and wmmenta are noted w the attached page. <br />$~Odti <br />The surface of the haul road was m good oonditiar, but the ditcbea were in and of maintenance. All of the Gght <br />use roads Deeded surface grading sad ditch maintenance. <br />The light use road leading ro the Swth F®/Manereh fm had created Iarge gullies which extended beyond the <br />disurbed area corridor. Tm such gullies were observed, sod are descrrbed as follows, traveling from the fan <br />installation north m the GEC Mine East Pit area: <br />1. A flat pottive of the road, approximatdY l00' below the fan has caused water to pool w the <br />roadway. Where drainage has escaped the roadway. s large gully has art through adjacent spoil <br />ridges created by pre-lave mining activities. <br />2. Approximately 10' deep. <br />3. Apprmomately 10' deep. <br />4. 3'~' deep, approximately 50' is lmglh. <br />S. 2'-3' deep, apprmdmately 50' in length. <br />6. Average of 3' doep, wind'mg downhill for approximately 100'. <br />7. Average of S' deep, winding downhill to the drainage bottom 75'-100'. <br />8. Approximately 3' deep, winding downhill tb the drainage bottom. <br />9. This gully is right at the base of the disturbed <br />arse cortidor. <br />Because these gullies were beyond the permitted area and the desigioeted disturbance area for the Southfield Miue, <br />Notiee of Violatieo No. CVA8-0OS was issued for firilure to wawa and maintain the fight use road providing <br />atxesa tb the South Fan installation ro caatrol or prevent erosive sad siltation sod failure to mews that the road will <br />not cause additional cenm'budans of eusPmded solids to runoff outside the permit arcs. The Divisive will generate <br />aerial photographs of the area to barer dorxrmeat the locative end extort of each gully. <br />Pond 1 held water sod was being discharged at n rate of approximately 45-30 gpm (operator estimate). Weser <br />being discharged loolud clear. The sediment level of Pond 1 watt well below the outlet level. Pond 2 held water <br />below the maximum allowable level. Pond 3 aLao held water well below the normal operating level. Panel 4 held <br />water far below the discharge level. Sediment had accumulated at the inla sad of the pond, but appeared to be <br />well blow the cleenotrt tstprirement point The weber level in Pond S was approximately tfs of the way up the <br />discharge pipe, but the pipe wan closed ro the pond was mt discharging. The inlet culvert ib the pond is rotting <br />slang the floor, causing flows to cat into the paad embadrmmdroad embartkmmt The culvert should be repaired <br />or replaced an noon as possble m insure stability ofthe pond and road. <br />Ditches within the portaUfiutilities yards ceeded to be maintained.
