The inspector noted on the previous page should e notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1 <br />2 <br />3 <br />The 9.7 acre permit area has not been identified with <br />permanent boundary markers as required by Rule 3.1.12 <br />Install permanent boundary markers along all sides of <br />the 9.7 acre permit boundary. <br />The operator has disturbed 3 acres outside the 9.7 acre <br />permit boundary. <br />Complete one of the following no later than October 22, <br />1997: <br />Option A <br />Submit an application for a conversion from a 110(2) <br />to a 112 permit. <br />Option H <br />Submit an application for financial warranty release <br />of at least 3 undisturbed acres.
