M
(Page 2)
MINE ID # OR PROSPECTING ID #: _ M-19A1-n72
INSPECTION DATE: 11/21101 INSPECTOR'S INITIALS: KAP
OBSERVATIONS
This inspection was conducted ,js part of the Division of Minerals and Geology's monitoring program. Present at the
inspection were Kate Pickford, DMG and David Simpson, Connell Resources. Weather at the time of the inspection
was clear, calm, and about 45 degrees.
A permit sign was posted at the entrance to the permit area. The boundaries of the permit site were marked. All
previous mining activity at the site appears [o have occurred within the permit boundary. Mining and some of the
reclamation of the site is complete. Gravel stockpiles remain on the south portion of the permit area. Land surface on
the site is nearly horizontal. Slopes along the pond shoreline are predominantly 5:1. There is a portion of the
shoreline along the eastern permit boundary, however, that is approximately 100 feet long with slopes ranging from
3:1, along a hill which cross cut; the area, to 1:1, where some shore erosion has occurred. The permit application
indicates that slopes on the reclaimed site will not exceed 5:1.
3. Vegetation at the site consists mainly of weeds. The presence of weeds without efforts to control them could be a
violation of 3.1.10 (6) of the Cor~structions Materials Rules and Regulations. Mr. Simpson advised that weed control
will be incorporated in the Spring of 2002. Mr. Simpson indicated that the gravel stockpiles will also be removed
during 2002 and reclamation activities continued . Although the pond shore has been seeded in the past, little
vegetation, with the exception of weeds, has been established along the shoreline. There was evidence at the time of
inspection, in the form of livestock footprints, that grazing has occurred on the site, which may have contributed to
the poor establishment of previously seeded flora. Mr. Simpson indicated that he was unaware of grazing activities at
the site.
4. The permit holder is required to incorporate weed control at the site by May 31, 2002, and send photos of the site
after the weeds have been eliminated to the Division of Minerals and Geology (DMG). Prior to release of the site, the
permit holder will be required to either backfill and grade the eastern area and shore of the pond to achieve 5:1
slopes, or to submit a technical revision to the permit indicating that some reclaimed slopes on the site will be 3:1 and
grade the eroded areas to conforrn to this guideline.
I & E Contact Address
NAME: David
OPERATOR: Connell Resources
STREET: 4305 Harmony oad
CITY/STATE/ZIP: Ft.
