This included Ditch C-1 <br />serving the facilities area for erosion problems ;Lack of adequate drainage <br />controls on the haulroad at triangle intersection crossing to pond E including <br />failure to maintain culvert; lack of ditch design for west side of road; and <br />unapproved culvert installation 1 culvert installation just south of culvert 2 and a <br />6 inch culvert at the loadout beneath the loadout conveyor - and MSHA safety <br />berm and constructed ,rip-rapped outlet in the berm do not show on approved <br />Map 16A ,Mine Area Surface Water Hydrology. Additional comments <br />concerning roads include maintaining ditch 5 when ground conditions allow and <br />evaluating the need for durable road surfacing when repair work is completed at <br />the triangle intersection. <br />NOV C-93-029 was issued by DMG for failure to have anon-clogging <br />dewatering device on sediment pond E. The water level in the pond was over <br />the discharge holes in the principal spillway riser pipe and the ponds surface <br />was entirely frozen. The pond was not discharging and the discharge holes <br />were also frozen closed. This was discussed with Mr. Patterson on site. <br />Pond F was discharging approx. 5gal./min. and was not sampled due to <br />completely clear discharge. Pond D was sampled on 4/1 at 11:35 am ,the <br />discharge culvert outlet had been uncovered that morning by a company <br />employee who was also sampling apparently. The outlet was under 3 feet of <br />
