• (Page 41 • <br />MINE ID # UR PROSPECTING ID # M-92-054 <br />INSPECTION DATE 10/5/95 INSPECTOR'S INITIALS WHE <br />The following problems IPBI endlor possible violations IPV) land suggested corrective actions) were identified during this inspection. <br />The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted <br />on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />Mine identification sign and affected area perimeter markers, required by Rule 3.1.12, were not observed. Rule 3.1.12111 states <br />that the operator shall post a sign at the entrance of the mine site, clearly visible from the access road, which displays the name <br />of the operator, the permit number, and a statement that a reclamation permit for the operation has been issued by the Colorado <br />Mined Land Reclamation Board. Rule 3.1.12121 states that the boundaries oT the affected area will be marked by monuments <br />or other markers that are clearly visible and adequate to delineate such boundaries. For 110 Limited Impact Operations, the permit <br />boundary shall be considered the affected area. <br />CORRECTIVE ACTION: Post and maintain mine identification sign and affected area perimeter markers until DMG approves <br />complete release of financial end performance warranties and termination of permit. <br />11 /24/95 <br />PB or PV PB <br />TOPICIS) SM <br />2. The operator needs to perform weed control measures on the affected areas of the operation. Rule 3.1.10161 states that weed <br />control shall be employed for all prohibited no:ious weed species, and whenever invasion of a reclaimed area by other weed <br />species seriously threatens the continued development of the desired vegetation. Weed control methods shall also be used <br />whenever the inhabitation of the reclaimed area by weeds threatens further spread of serious weed pests to nearby areas. <br />CORRECTIVE ACTION: Perform weed control measures as required by Rule 3.1.10(61. <br />1 1 /24195 <br />PB or PV PB <br />TOPICISI RV <br />3. Fuels stored on site, not stored in a controlled manner to ensure disturbance to the prevailing hydrologic balance be minimized <br />as required by Rules 3.1.6 and 3.1.7. One 500-gallon elevated fuel tank was located on site. Evidence of minor leakage was <br />observed et the refueling hose nozzle, tank appeared intact, no evidence of leakage from tank. All fuels, lubricants, and chemicals <br />need to be stored in a eonVOllad manner, at a designated location, with adequate spill prevention and spill containment controls. <br />Spill eoMainment controls typically include measures such as construction of a containment berm surrounding fuel tank or fluid <br />storage eras. Berm surrounding the fluid storage area should enclose sufficient area to ensure containment of volume equivalent <br />to tank capacity. Containment arses should be lined with an impermeable liner to ensure spilt fuel or fluids not escape end <br />degrade quality of ground and surface waters (Rules 3.1.6 and 3.1.71. <br />CORRECTIVE ACTION: Properly store ell fuels and lubricants in a controlled manner.
