• (Page 21 <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 10/5/95 <br /> <br />M-92-054 <br />INSPECTOR'S INITIALS WHE <br />OBSERVATIONS <br />This complete inspection was conducted by Wally Erickson from the DMG field office in Durango. DMG inspector scheduled the <br />inspection with Mr. John Gilleland for 10/5/95 at 10:OOam. No mine personnel were present during the scheduled inspection. After <br />the inspection, DMG inspector contacted Mr. John Gilleland, he indicated that he had intended to attend DMG inspection, but had <br />experienced equipment problems at another job that required his attention. The Ignacio Pit is located approximately 1.5 miles north <br />of the New Mexico border and 0.25 miles west of highway 172, on the south side of La Boca Canyon in the SW 1 /4 of the SE 1 /4 <br />of Sec[ion 16, T32N, R6W of the NMPM in La Plata County. The 110(21 permit was issued on 7/74/92 for reclamation only, no <br />active mining to occur, for 4 acres of affected area. DMG holds financial warranty in the form of a certification of deposit with <br />United Bank of Ignacio in the amount of 55000.00. <br />Mine identification sign and affected area perimeter markers were not observed as required by Rule 3.1.12. <br />Vehicles and equipment located on site included: <br />• Yellow metal shed approximately 6' wide and 20' long, mounted on skids end labeled SMC Contracting Company, Farmington <br />N.M. DMG inspector did not enter shed. Shed contained three 55-gallon drums, one drum had a pump mounted on top, one <br />small compressed air tank, and assortment of PVC pipe sections and fittings; <br />• One hydraulic excavator, CAT 225, labeled FESCO Inc. 326-2985,parked in a trough adjacent to stockpile of gravel with bucket <br />in the stockpile, excavator had been recently utilized to load material from stockpile into trucks; <br />• One backhoe; <br />• One tandem exit end dump truck, approximately 10 cu yd capacity, labeled FESCO Inc. 326-2985, New Mexico license number <br />IR E0850; <br />• One 500-gallon elevated fuel tank with hose and nozzle, evidence of minor diesel fuel spilt at nozzle. Fuel tank was located <br />beside the hydraulic excavator; <br />• Stockpile of 12" diameter metal pipe; and <br />• Two stockpiles of aggregate material, larger stockpile containing approximately 600 cu yds, smaller stockpile containing <br />approximately 100 cu yds, weeds were established on the stockpiles. <br />The affected area appeared to be contained within the approved 4 acre limit. A previously mined area of approximately 1 acre in <br />size was located immediately east of the permit area. Disturbances at the previously mined area occurred prior to permit issuance <br />and is not associated with this permit. Backfilling and grading of the approved affected area was complete except for one cut slope, <br />approximately 130' long and 5' high, extending from the stockpile to La Boca road, along the west side of the affected area. <br />Vegetative cover on the backfilled slopes was sparse consisted of rubber rabbit brush, various grasses, and weeds. The operator <br />needs to perform weed control measures on the affected areas of the operation. Rule 3.1.1016) states that weed control shall be <br />employed for ell prohibited noxious weed species, and whenever invasion of a reclaimed area by other weed species seriously <br />threatens the continued development of the desired vegetation. Weed control methods shell also be used whenever the inhabitation <br />of the reclaimed area by weeds threatens further spread of serious weed pests to nearby areas. A list of noxious and problem weeds <br />end control techniques for these species een be obtained from the County Extension Service, the County Weed Control District <br />Office, or the local office of the Soil Conservation Service. <br />An irrigation pips was being installed approximately 100 yards west of the permit boundary. <br />A newly constructed dozer toed exited the northeast corner of the permit area end proceeded north across Le Boca Canyon. The <br />dozer road did not appear to be related to activities approved for this permit. Additional unpermitted disturbances were located <br />within La Boea Canyon, immediately east of the permit area and On the north side of La Boca road. Vegetation had been stripped <br />and alluvial material had been removed from the floor of La Boca Canyon. Cut slopes, resulting from the excavation, appeared to <br />be has high as 6'. Disturbed area appeared to be less than 0.5 acres. <br />
