• (Page 3) • <br />MINE ID # OR PROSPECTING ID # M-80-121 <br />INSPECTION DATE 11/21/95 INSPECTOR'S INITIALS CLK <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />DMG recommends that the Operator bring more topsoil to Jan. 21, 1996 <br />the site, or submit plane for amending the topsoil due PB or PV PB <br />to a shortfall of necessary material within 60 days. TOPIC(S) TS <br />It is recommended that the Operator submit a technical Jan. 21, 1996 <br />revision for a plan for site e~tabilization during PB or PV PB <br />operations shutdown (when a temporary ceasat ion of TOPIC(S) RS <br />operations notice should be sent to DMG). Reclamation <br />efforts must continue in accordance with Rule 3.1.3 of <br />the Colorado Mineral Aulea and Regulations: "All <br />reclamation shall be carried to completion by the <br />Operator with all reasonable diligence, and each phase <br />of reclamation shall be completed within five (5) years <br />from the date the Operator informs the Board or Office <br />than such phase has commenced, unless extended by the <br />Board or Office". Should the Operator wish the site to <br />be reclassified ae an "intermittent" operation, a <br />technical revision must be submitted to DMG designating <br />this pit as such. <br />PB or PV <br />TOPIC(S) <br />
