(Page 4) • <br />MINE ID # OR PROSPECTING ID # M-83-084 <br />INSPECTION DATE August 15, 1997 INSPECTOR'S INITIALS TAS <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. Results of permit information and a recent site survey October 27, <br />indicate that the site is out of compliance with the 1997 <br />mine plan requirements of 29 acres of affected land, a PB or PV PB <br />maximum pit disturbance of 8 acres, and a maximum depth TOPIC(S) MP <br />of excavation of 100 feet. This inspector estimates <br />there are currently 60 acres of affected land, 9.8 <br />acres of pit disturbance, and a maximum highwall height <br />of 131 feet. <br />Perform one of the following corrective measures within <br />60 days: <br />A. Submit an application for a 4th permit amendment <br />with a revised mining and reclamation plan and current <br />maps to account for all disturbances as noted by the <br />Division. <br />B. Perform reclamation to the extent that there are no <br />more than 29 acres of affected (disturbed) land of <br />which no more than 8 acres is pit disturbance, and <br />backfill the south end of the pit to the extent that <br />the total highwall height is no more than 100 feet. <br />2. The operator has begun the construction of a second September 26, <br />processing plant without prior approval by the 1997 <br />Division. There is no record of such construction in PB or PV PE <br />the mine plan and no plans for reclamation. TOPIC(S) MF <br />Submit an application for a technical revision to the <br />permit within 30 days which includes the type of <br />material used for construction, plans for reclamation, <br />cost of demolition, and the facility dimensions or the <br />volume of concrete, steel, and other materials used in <br />construction. <br />3. The primary commodity to be mined is unknown, both sand September 26, <br />& gravel and gold are listed. 1997 <br />PB or PV PB <br />TOPIC(S) AR <br />Submit a written statement of the primary commodity to <br />be mined at the Snowstorm Placer, ie: either sand ~ <br />gravel or gold <br />
