The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The mining disturbance across the site is not following the sequence of mining 5/15/01 <br />phases as set out in the approved mining plan. PB or PV: PB <br />TOPICISI: MP <br />CORRECTIVE ACTION: The operator must request a technical revision ITRI to the , _ <br />mining plan, which accounts for the existing condition and future expansion. This TR <br />request must be received by the date shown at right. <br />The highwall configuration is vertical, though the approved plan requires that they be 5/15/01 <br />mined at 2:1 slopes. PB or PV: PB <br />TOPICISI: MP <br />CORRECTIVE ACTION: The operator must request a TR to the mining plan, which _, : _ <br />accounts for this different method of mining. (Please note that this TR may be <br />included with the one required in PB 111 above.) This TR request must be received by <br />the date shown at right. <br />3. There is a wash pond in the floor of the pit which is impounding groundwater, though 5/15101 <br />the permit does not include approval for that activity, in that location, or for exposure PB or PV: PB <br />of groundwater. TOPICISI: HB _, , _ <br />CORRECTIVE ACTION: The operator must EITHER completely remove the pond by <br />backfilling the pond to the grade of the surrounding floor; OR request a TR which <br />includes revisions to the mining plan for a wash pond. If a TR to include the pond is <br />submitted the appropriate documentation that exposure of groundwater is approved <br />must be provided. <br />Evidence that the earthwork is completed, OR submittal of the TR request, must be <br />received by the date shown at right. <br />4. The diesel fuel tank adjacent to the wash pond shows signs of fuel spillage on the <br />ground surface, potentially contaminating groundwater. <br />CORRECTIVE ACTION: The operator must promptly remove the fuel tank and all <br />contaminated material from the proximity of the groundwater. Fuel contaminated <br />materials must be properly disposed of as described in the report. This activity must <br />be oerformed oromotly!
