• (Page 3) • <br />MINE ID / OR PROSPECTING ID ~ M-81-276 <br />INSPECTION DATE 06/12/96 INSPECTOR'S INITIALS AJW CBM <br />The following problems (PB) and/or possible violations (PV) (and eugg~aeted corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered b)r the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The operator was failing to protect the hydrologic )8/16/96 <br />balance (34-32.5-116(h)) and was not following the PB or PV: PH <br />approved mining plan, specifically exhibit G states TOPIC(S): H~ <br />that a berm will be maintained along the west aide of MP, SC, SF, <br />the pit area to prevent surface water from entering <br />into the Yampa River and a drainage trench had been <br />installed through the berm to allow surface water from <br />the pit to enter the Yampa River. <br />Corrective actions are to: 1) re-install the berm to <br />prevent drainage from entering the Yampa River or; 2) <br />Submit a technical revision to the permit to <br />incorporate this change in the mining plan. This <br />revision shall include all appropriate discharge <br />permit e, amended maps clearly showing the drainage <br />channel, and appropriate changes to "Exhibit G" <br />describing how surface water will be handled ae mining <br />progresses at this operation. If option f 1 ie <br />followed, a photograph clearly showing that the berm <br />has been re-installed shall be submitted prior the <br />expiration of the corrective action date.
