Eastside Mine; pg 3 <br />We next reviewed the permittee's permit and monitoring records <br />which are being maintained at the Garfield County Courthouse in <br />Glenwood Springs. Eastside Coal Company, now an absentee <br />permittee located in San Antonio, Texas, has done a good job of <br />accurately maintaining their records. Records reviewed included: <br />The TR approvals noted above; TR No. 12 " Change in Water <br />Monitoring " approved May 2, 1995, which documents possible <br />revisions to the approved water monitoring plan one year after <br />the completion of backfilling and grading activities; A spring <br />and seep survey is required during June of each year; Sediment <br />pond inspection reports were current through the first two <br />quarters of 1995 ( The permittee needs to provide certified <br />copies of the inspections conducted during February and April <br />1995 ); NPDES Permit COG-850019 was amended 10/21/94 to add the <br />Storm Water Management Plan and the Annual Report for the plan <br />was submitted on 1/25/95; NPDES reporting is current in the <br />records through the first quarter of 1995;the 1994 Annual <br />Hydrology Report was submitted on 1/25/95; a 1995 subsidence <br />report was submitted on 5/1/95 covering the period through April <br />1995; and finally the Liability Insurance Certificate was <br />reviewed and expires on 9/1/95.
