Y . • <br />~ (Page'2) <br />MINE ID # OR PROSPECTING ID # M-90-057 <br />INSPECTION DATE 11/16/00 INSPECTOR'S INITIALS ESC <br />OBSERVATIONS <br />This was an inspection of the Hopemore & Leadville Mine conducted by Erica Crosby of the <br />Colorado Division of Minerals and Geology. Scott Hazlitt representing Leadville Mng & <br />Milling Corp was present during the inspection. Weather conditions on site were clear and <br />cold. Approximately 1 foot of snow was present on site. <br />The mine site is located approximately 3.3 miles east of Leadville and the mill is located <br />roughly 2 miles south of Leadville off of Highway 24. The total disturbance for the mine and <br />mill area is 8.5 acres; of which 6.6 acres are applicable to the mill site and 1.9 acres are <br />applicable to the mine area. The mining and reclamation permit was issued in 1990. <br />During the inspection the operator made a copy of the DMG file for the Hopemore & Leadville; <br />specifically the permit application and the correspondence file. <br />Hopemore & Xunter Shaft Site <br />No mining was taking place during the inspection, but the site remains in active status. <br />Small ore piles noted in the previous inspection were hauled down to the mill or graded in <br />place at the mine area. It was noted during the previous inspection that the low-grade ore <br />consist of lead and zinc and the high-grade ore contains up to 70~ pyrite. <br />A mine identification sign and permit boundary markers were noted on site. <br />During the last inspection, the operator was actively reclaiming old abandoned shafts and <br />subsidence features located on property owned by Leadville Mining & Milling Corp. The <br />operator indicated that this procedure would continue on the property. Attached is a copy <br />of the General Hid Specifications prepared by the State of Colorado, Division of Minerals and <br />Geology for your use. Please note that the operator will need to submit a brief letter to <br />the Division explaining any reclamation outside the permit area. This is to ensure that the <br />Division is aware of the activity and does not inadvertently deem the activity as part of the <br />mining operation. <br />The following problems were noted at the Hopemore & Hunter Shaft Site that need to be <br />addressed; <br />1. The permit makes note of an exit shaft, but the maps do not include the location of the <br />Hunter shaft. The operator will need to clarify the permit text and maps to include the <br />affected area of the Hunter Shaft. Permit and map clarifications must be submitted to the <br />Division in the form of a Technical Revision by January 19, 2000 (no application fee <br />required). Please be aware that if this area exceeds the 8.5-acre permit area, a new <br />permit application will need to be filed (please note that an amendment to the permit is <br />not allowed under a Hard Rock/Metal Mine 110(2) permit). Attached is an application form <br />for your use if filing a new permit is needed. <br />2. The operator is reclaiming old abandoned mining features on property owned by Leadville <br />Mining & Milling Corp. The operator will need to submit the Division a letter stating the <br />location of the reclaimed features and that only inert material will be used. <br />MiZZ Site <br />No milling was taking place during the inspection. According to the operator milling ceased <br />in late 1999/early 2000. The mill site consists of the mill building, tailings pond, storage <br />facility and waste piles. <br />
