• (Page 2) • <br />MINE ID # OR PROSPECTING ID # M-91-037 <br />INSPECTION DATE June 11, 1997 INSPECTOR'S INITIALS TAS <br />OBSERVATIONS <br />i. A site inspection was made in response to a recent request for a Succession of Operator's <br />submitted by Mr. Tony Kimmick of Monarch Stone Co. Boyd Astemborski of Gold Park Recovery <br />& Reclamation is the current permittee and operator of the site. <br />2. An entrance sign was posted; however, there were no markers to clearly identify the 6 <br />acre permit boundary. Therefore, the site is not in compliance with Rule 3.1.12 pertaining <br />to signs and markers. Accordingly, the Division requests that the current operator clearly <br />identify the permit boundary within 30 days and prior to approval of the permit transfer. <br />Please see page 3 for details. <br />3. Based on the current disturbance, this inspector has calculated a reclamation cost of <br />$5,600 in order to complete the reclamation plan. (Please note that this cost does not <br />include reclamation of the pre-law highwall disturbance along the southeast portion of the <br />permit area) Therefore, the successor operator will need to provide a $5,600 financial <br />warranty or the current operator will need to perform at least $600 worth of reclamation <br />tasks prior to approval of the permit transfer. Attached is a copy of the reclamation cost <br />calculated by the Division. Since the site is slightly underbonded, the Division considers <br />this issue to be a problem that requires corrective actions. Please see page 3 for details. <br />4. There was evidence of spilled waste oil on the ground near the pre-law highwall in the <br />southeast portion of the permit area. This inspector notes this concern as a problem that <br />should be resolved within 30 days and prior to approval of the permit transfer to Monarch <br />Stone Co. The Division requests that Mr. Astemborski properly dispose of all waste oil, <br />contaminated cobbles, and/or other petroleum products prior to approval of the permit <br />transfer to Monarch Stone Co.
