<br /> <br />(Page 2) <br />MINE ID # OR PROSPECTING ID: #M-2000-039 <br />INSPECTION DATE: 5/8/00 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS: JCS <br />This inspection was conducted in order to check compliance of the operation with the <br />requirements of the Act, Rules and the conditions of the permit. The site, operated by Delhur <br />Industries, Inc., was just concluding active operation for the day at the time of the <br />inspection. The operator was represented by Randy Dawley during most of the inspection. <br />There was no ID sign posted for the operation. This is a compliance problem that will be <br />included on Page 3 of this report. <br />The access road to the pit is new, included in the approved permit area and marked by the <br />limits of its disturbance. Boundary markers for the pit, which comprises the bulk of the <br />permit area, were identified for this inspector by the Company Representative. <br />The access road must cross a small, natural drainage in order to reach the pit. The last part <br />of the access road consists of a short steep climb into the pit area itself. There is a small <br />berm constructed in the drainage which the Company Representative indicated was to assist in <br />controlling and directing runoff from the pit draining down the last steep portion of the <br />access road and into the natural water course. This is not the storm water control system <br />that was approved with the application. Runoff from the pit area was to be collected behind <br />a berm constructed inside the boundaries of the permit area and maintained to keep storm <br />water separate from arroyo flow. (A copy of the pertinent part of the approved Exhibit E <br />Mining Map is enclosed for your review.) This is a compliance problem that will be included <br />on Page 3 of this report. <br />The pit has only recently been opened and disturbance at the site is relatively limited. <br />Reportedly, materials from the pit are being transported to the Summitville Mine for use in <br />treating acid mine drainage. Equipment on site included bulldozers, loader and trucks. In <br />addition, although there was no significant associated surface disturbance at this time, <br />there was a portable toilet and some boxes of supplies placed across the natural drainage <br />from the pit and, therefore, outside of the approved permit area. Since these items, <br />materials, etc. are directly associated with the mining operation, they should be removed and <br />placed inside the approved permit area in order to prevent the occurrence of any mining <br />related surface disturbance outside the permit area and a possible violation. <br /> Cc: <br />I & E Contact Address <br /> ^ CE <br />NAME: Rick Hurworth ^ BL <br />OPERATOR: Delhur Industri es.
