The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVH ACTIONS <br />DATE <br />1. Rule 3.1.5 requires the operator to submit information regarding _ <br />placement of inert material on site. In addition, Rule 3.1.6(1) PB or PV:PB <br />states disturbance to the prevailing hydrologic balance to the TOPIC(S):HB, BG <br />quality of water in the surface and groundwater systems shall be <br />minimized both during mining and reclamation. <br />CORRECTIVE ACTION: <br />• Specify the nature of the inert material. <br />• Explain the Summit County's role regarding the signed affidavit <br />certifying that the material is clean and inert. <br />• Clarify how the operator verifies on site that material is clean <br />and inert, and that wood and wood products are not backfilled <br />into the pit/pond. <br />• Ensure that any concrete disposed in the pit/pond must be in <br />a hardened state for at least sixty days. Please describe <br />the handling and dumping procedures to ensure this <br />requirement is met. <br />• Verify to the Division that the groundwater has not been <br />impacted or polluted as a result of the material placed in <br />the pit/pond on August 22, 2000. The operator will need to <br />sample the pit/pond and an upstream location for pH and <br />Electromagnetic Conductivity (EC). <br />
