MINE SITE EVALUATION INSPECTION REPORT Fruita 1 ~ 2 GENERAL COMMENTS PAGE 6 This inspection was conducted with Jim Herron of OSM, Steve Engle of Dorchester Coal Company and Ira Dutton of OSM. The mine is currently inactive. An extensive mine fire is burning in the Anchor (Cameo) seam at mine No. 1. This fire may extend into Utah. Mine No. 2 is located at the lower Fruita Seam. Two portals have been driven back approximately 1,000 feet. PERMITS No one at the mine knew whether any permits or records were kept at the mine office. After some discussion with Dean Massey of Dorchester Coal Co. in Denver, he agreed to send me copies of all applicable records. He also informed me that these were at the mine office the day of our inspection. The following items were sent for review: 1. A Development and Extraction Mining Permit, 79-160, issued by MLRD on 9/5/79. 2. A surety bond in th rado. 3. Air Quality Permits: C-13,118 - Initial Department of Health C-12,540-1 - Initial C-12,540-2 - Initial C-12,540-3 - Initial C-12,540-4 - Initial C-12,540-5 - Initial C-12,615 - Initial e amount of approval ~ on 2/18/81. approval on approval on approval on approval on approval on approval on $110,000.00 is held by Colo- uas issued by the Colorado 8/14/79 8/14/79 8/14/79 8/14/79 8/14/79 1/23/80 4. A final NPDES Permit No. CO-0036609 was issued by the Colorado Department of Health on 12/3/79. Authorization to discharge was given on 11/13/79 and expired 12/31/80. Discharge pt 001 is at the west sediment pond. NPDES Permit No. CO-003960 was issued by CDOE1 covering this same discharge point on 6/24/81. The expiration date is 3/31/86. A renewal NPDES Permit No. CO-0036609 was issued on 7-30-81 and expires 3-31-86. This permit covers discharge points 001 and 002 located at the west and east sediment ponds, respectively.
