The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />The quarry access road exhibits marble fines, which may have originated in the <br />permitted area, transported downhill through uncontrolled sedimentation. <br />CORRECTIVE ACTION: The operator must provide information as to the origin of <br />(i.e., deliberate placement of or uncontrolled deposit of) the fines on the roadway. <br />Additionally, the operator must provide information pertinent to any stormwater <br />control structures or practices to be employed at the site, which items may be <br />included under a discharge or stormwater permit issued by CDPHEIWOCD. <br />This written information must be submitted to the Division by the date date shown at <br />right. If such information indicates an uncontrolled release, this matter may be <br />pursued as a possible violation before the Board.
