ORIGINAL-PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~~-~nn3-n~I <br />INSPECTION DATE: -~A/29/47 INSPECTOROS INITIALS; _~K- <br />OBSERVATIONS <br />1) The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2007 and 2008. Present during the inspection was <br />Miss Angela Belantoni the operator's consultant and Mr. Mike Ausuburn, of Pioneer <br />sand, the operator of the site were present. <br />2) Site is a phased 112 operation covering 930 acres with a maximum disturbance of 60 <br />acres at any give time. <br />3) Site was active at the time of the inspection in Phase III of the permit area. A <br />couple of front end loaders, a dozer and screen were noted in the active mining <br />area. Only screening operation is utilized to process the aggregate. <br />4) Large quantities of processed stockpiled material was noted through out the affected <br />area. <br />5) The active high wall approximates 550 feet in length and between 10 and 15 feet in <br />height and was stable. <br />6) Adequate berm was noted around the affected area to protect drainage from entering <br />the dry gulches. No Erosion feature of significant size were noted at the time of <br />the inspection. <br />7) There was no activity in Phase I, however, the acres disturbed had not been <br />reclaimed and stockpiled processed material was noted at the time of the inspection. <br />According to the annual report, operator states once the processed stockpiled, <br />material is removed, reclamation will commence. The maximum allowable affected acres <br />is 60 acres at any given time. No actual measurements of the affected AREAS was <br />conducted at the time of the inspection. In order to make sure the operator has not <br />exceeded the allowable affected acres at any given time, the Division request the <br />operator to provide a survey of the affected acres in both Phase I& III. Once the <br />new maps are provided showing the actual affected acres, if the areas affected <br />exceeds 60 acres, DRMS, will recalculate the needed warranty.
