The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />DATE <br />1. The Division estimated the cost to reclaim the B & G Stone Quarry PB or PV: <br />to include backfilling, grading, revegetation and mobilization/ TOPIC (S):FN <br />demobilization to and from the job site. The Division's estimate Within 60 Davs <br />based on the site conditions and permit requirement totals <br />$7,300.00. <br />CORRECTIVE ACTION: <br />Post an additional $4,300.00 financial warranty, complete some <br />reclama~=ion on site, or revise the reclamation plan within 60- <br />days of receipt of this report. <br />2. The Division is not clear of who is the current operator) PB or PV: <br />permit take of the B & G Stone Quarry (B & G Stone or Rocky Top TOPIC (S):AR <br />Stone). Within 60 Davs <br />CORRECTIVE ACTION: <br />The operator must inform the Division as to whom is the <br />current operator(permittee for the B & G Stone Quarry, and <br />file a Transfer of Mineral Reclamation Permit and Succession <br />of Operzctors Application, if applicable, within 60-days of <br />receipt of this report. <br />(Page 3) <br />
