June I I, 1997 <br />Hayden Gulch Terminal, Inc. <br />1475 Pine Grove Rd., Suite 207 <br />Steamboat Springs, CO 80488 <br />Hayden Gulch Loadout <br />Complete joint inspection <br />June 3, 1997 <br />Participants: <br />Tom Petrosky, Hayden Gulch <br />Joe Dudash, DMG <br />Tom Kaldenbach, DMG <br />Henry Austin, OSM <br />I participated in a complete, joint ,oversight inspection of the I-Iayden Gulch coal loadout facility <br />with the above participants. Tliis site was selected by the Office ot'Surface Mining (OSM) and <br />Colorado Division of Minerals and Geology (DMG) joint oversight team for evaluation during the <br />1997 evaluation year. The primary Focus ot'the inspection was oft=site impacts and reclamation <br />success. Weather conditions immediately prior to the inspection Itad led to flooding along the <br />Yampa and Elk Rivers and the site ground conditions included standing water and saturated soils <br />in places. <br />We began the inspection with an office records review at the above address. Mr. Dudash letl a <br />thorough review of required records which included the following <br />The current approved permit document effective 5/32/9; through 5/22/95; DMG's initial <br />mid-term review findings to the permittee containing one stipulation (requiring an AVS check on <br />a Kentucky violation ), the current surety bond Held by Aetna No. G45100808349BLA for <br />$862,120 and effective 4/5/96; liability insurance coverage meets minimum requirements and is <br />effective 3/26/97 through 10/01/97; sediment pond inspection reports are current through 5/l6/97 <br />(the permit is being revised to more accurately retlecl the current storage capacities of the rail <br />loop pond); DMR's are current through 3/ 31/97, the permittee tiled a renewal application dated <br />12/19/96 for NPDES Permit COG-550005 and the current permit remains in effect, there is a <br />2/25/93 storm water management plan which retlects current site conditions, both the annual <br />hydrology report and annual reclamation report are current through 2/97; sediment pond <br />certifications are dated 12/14/92 for pond 2 and .'+/5/93 for pond I; and haulroads are existing <br />structures and the permit contains a statement indicating the existing designs will meet the <br />required performance standards. Small area exemptions include the three topsoil stockpiles <br />and the rail loop outslopes; and alter Hate sediment control areas include the office trailer and <br />power substation. These areas are approved in the permit with appropriate sed cad <br />demonstrations and/or designs for alternate sediment control <br />
