• (Page 2) • <br />MINE ID'~# OR PROSPECTING ID # M-90-057 <br />INSPECTION DATE 10/16/98 INSPECTOR'S INITIALS ESC <br />OBSERVATIONS <br />This was an inspection of the Hopemore & Leadville Mine conducted by Erica Crosby of the <br />Colorado Division of Minerals and Geology. Donald Wilson representing Leadville Mng & <br />Milling Corp was present during the inspection. The Hopemore & Leadville Mine is a <br />Designated Mining Operation (DMO). Weather conditions on site were clear and cool. Ground <br />conditions were dry. <br />The mine site is located approximately 3.3 miles east of Leadville and the mill is located <br />roughly 2 miles south of Leadville on Highway 24. The total disturbance of the mine and mill <br />sites is 8.5 acres of which 6.6 acres are applicable to the mill site and 1.9 acres are <br />applicable to the mine area. The mining and reclamation permit was issued by the Division in <br />1990. The Division last inspected the site August 30, 1994. <br />The site has not actively mined to date. According to the operator, MSHA requires that an <br />exit shaft be constructed prior to mining. The Hopemore Shaft is the main mine entry and the <br />Hunter Shaft is the exit shaft currently under construction. The Hopemore shaft is at an <br />elevation of 11,700 feet and extends down to 10,800 feet. Both shafts have an opening of 4 <br />34 feet wide and 9 feet long. The shafts are securely locked within the hoist buildings. The <br />permit makes note of an exit shaft, but the maps do not include the location of the shaft. <br />The shaft disturbance appears to fall within the 1.9 acres of disturbance stated for the mine <br />area, but not within the permit boundaries. The operator will need to clarify the permit <br />text and maps to include the affected area of the Hunter Shaft. Permit and map clarifications <br />must be submitted to the Division within 60 days of the receipt of this report. Mine signs <br />were posted at the site, but permit boundary markers could not be located. The operator will <br />need to post the boundary markers and submit to the Division photographs that the markers <br />were posted within 60 days of this inspection report. <br />The mill site consists of a mill building and a tailing disposal area. No tailings have been <br />disposed in the area to date. Amine identification sign was posted and the permit boundary <br />area was delineated by fences. <br />The Division currently holds two reclamation bond in the amount of $11,000.00. The <br />reclamation plan requires that the two shafts be sealed with a permanent bulkhead seal and <br />backfilled with 20" of fill and at least 12" of topsoil. All structures and facilities will <br />need to be removed, and the mine waste rock be graded, topsoiled and seeded.
