The above-mentioned site has had one or more problems noted that, according to Division of Reclamation, Mining and
Safety (Division) records have not yet been resolved. According to our records, the following problem{s) was{were) to be
resolved on or before May 30, 2007. Listed below are descriptions and corrective actions for the problem(s). Please resolve
the corrective actions and provide evidence of such resolution on or before October 31, 2007.
Please note that if the corrective action(s) are not properly addressed, and evidence provided to the Division by the date
provided above, the listed problems could become escalated to the level of possible violations and would then be referred to
the Mined Land Reclamation Board for further action.
If the problem(s) has have) already been remedied, please provide evidence of its(their) completion.
Recycled blacktop stockpile onsite, which is not a part of the approved mining and reclamation plan.
CORRECTIVE ACTION:
Submit a notice on the use and storage of the blacktop to the Division pursuant to section 3.1.5(9) of the Mineral
Rules and Regulations- including the following information on or before bfay 30, 2007:
* Written documentation of the intended use of this asphalt.
* Written documentation of the wishes of the landowner to use this material for road maintenance and
that the material be left onsite after reclamation and final surety release.
* Written statement committing to not bury the asphalt or in any way use this material as backfill or
pond fill.
* A narrative and/or a map that describes/illustrates the approximate location of the asphalt stockpile.
* The approximate volume of the asphalt.
* Signature of the operator and landowner.
