(Page 31 <br />MINE ID # OR PROSPECTING ID # M-77-376 <br />IRIS PECTION D~TE 2/8/99 INSPECTOR'S INITIALS AJW/JCS <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br /> PROBLEMS/POSSIBLE VIOLATIONS <br /> AND <br /> CORRECTIVE ACTIONS <br /> CORRECTION <br /> DATE <br />1. There was not a mine identification sign posted at the 03/18/99 <br />public entrance to the mine site as required by Rule PB or PV: PV <br />3.1.12(1)(a),(b) & (c), and the affected area TOPIC(S): SM <br />boundaries were not clearly marked by monuments or <br />other markers as required by Rule 3.1.12(2) <br />Corrective actions are to: <br />1) Place a mine identification sign at <br /> the entrance to the mine site that <br /> contains at a minium: the name of the <br /> operator and "MLRB Permit # M-77- <br /> 376". <br />2) Mark the boundaries of the affected <br /> area with markers that are clearly <br /> visible and adequate to delineate <br /> such boundaries. <br />3) Contact our office when this is <br /> completed so that we can conduct a <br /> follow-up inspection and submit a <br /> photograph to our office which <br /> clearly shows that the identification <br /> sign and affected area boundary <br /> markers have been installed. <br />
