The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE 1. There was not a mine identification sign posted at the 03/18/99 public entrance to the mine site as required by Rule PB or PV: PV 3.1.12(1)(a),(b) & (c), and the affected area TOPIC(S): SM boundaries were not clearly marked by monuments or other markers as required by Rule 3.1.12(2) Corrective actions are to: 1) Place a mine identification sign at the entrance to the mine site that contains at a minium: the name of the operator and "MLRB Permit # M-77- 376". 2) Mark the boundaries of the affected area with markers that are clearly visible and adequate to delineate such boundaries. 3) Contact our office when this is completed so that we can conduct a follow-up inspection and submit a photograph to our office which clearly shows that the identification sign and affected area boundary markers have been installed.
